Nosocomial outbreak of Blastoschizomyces capitatus associated with contaminated milk in a haematological unit

2002 年 7 月、血液内科部門で好中球減少を呈する患者 4 例からブラストシゾミセス・カピタツス（Blastoschizomyces capitatus）が検出された。患者 2 例は播種性の感染症のため死亡し、他の 2 例は口咽頭に保菌がみられた。真菌の院内感染が疑われ、疫学的調査と環境調査を実施した。想定される真菌感染の感染源を特定するため、患者間の疫学的関連を調査した。全患者の監視培養を実施するとともに、空気、無生物表面、食品、血液製剤、および化学療法剤の環境培養を行った。患者間の直接接触感染は認められず、B. capitatus は朝食用の牛乳を配膳するために使用されていた魔法瓶のみから検出された。4 種類の異なる分子タイピング法（パルスフィールド・ゲル電気泳動法、DNA 制限酵素分析、randomly amplified polymorphic DNA（RAPD）法、および 1 つのミニサテライト DNA または 2 つのマイクロサテライト DNA に特異的な単一プライマーを用いた PCR フィンガープリント法）により、すべての分離株を比較した。いずれのタイピング法でも、牛乳用魔法瓶の菌株と臨床菌株とを遺伝学的に識別することはできなかった。牛乳用魔法瓶を病院の全部門から回収したところ、それ以降は B. capitatus は検出されなかった。本研究の結果から、牛乳配膳用魔法瓶に由来する B. capitatus 単一株のクローンの伝播が、今回の血液内科部門での院内アウトブレイクの原因であったことが示唆される。
