Meticillin-resistant Staphylococcus aureus (MRSA) contamination of ambulance cars after short term transport of MRSA-colonised patients is restricted to the stretcher

入院患者を搬送する救急車内の表面は、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）汚染のリスクが高い。本研究では、MRSA 保菌・感染患者の搬送直後の救急車における MRSA の有無を解析した（ストレッチャーの 2 か所、内壁の 3 か所）。搬送 100 件のうち、搬送時間 20 分未満という組み入れ基準に合致した 89 件について、詳細な解析を行った。8 台の救急車（9％、90％信頼区間 4％ ～ 14％）が汚染されていた。搬送時間 11 ～ 20 分での汚染率は、より短い搬送時間である 1 ～ 10 分と比較して高くはなかった。MRSA が検出されたのはストレッチャー（ヘッドレストおよびハンドル）のみであった。車内の壁は汚染されていなかった。結論として、救急車には短時間の搬送であっても MRSA 汚染が認められた。MRSA 陽性患者の短時間の搬送後の消毒は、患者の位置のごく近い範囲の表面にのみとするべきである。MRSA の告知のない搬送 60 件の連続調査では MRSA は認められなかったが、メチシリン感性黄色ブドウ球菌が救急車 12 台から検出され、主な部位はストレッチャーのハンドルとヘッドレストであった。この結果は、患者の MRSA の状態に関係なく、患者周囲の表面の消毒が重要であることを示している。
