Inappropriate intravascular device use: a prospective study

血管内カテーテルによる合併症は臨床的に重要であるにもかかわらず、入院患者における不適切な血管内カテーテルの使用については十分な特性評価が行われていない。今回の前向き観察研究の目的は、適切な血管内デバイスの使用の定義、血管内デバイスが不適切に使用される頻度の推定、および入院患者に対する不適切な使用と関連するリスク因子および転帰の評価である。2007 年 10 月から 12 月に入院した患者 436 例の合計入院日数は 2,909 日、血管内デバイスの使用は 876 件であった。合計 3,806 カテーテル日が記録され、このうち本研究の基準に基づいて不適切と判定されたのは 1,179 カテーテル日（31％）であった。ロジスティック回帰分析からは、年齢、カテーテル使用本数、およびカテーテル留置期間が、不適切なデバイス使用のリスク因子であることが示された（P ＜ 0.05）。不適切な使用は、集中治療室への入室の増加（P ＜ 0.05）および入院期間の延長（適切な使用 4.9 ± 4.3 日対不適切な使用 8.5 ± 12.6 日、P ＜ 0.05）と強く関連していた。中心静脈カテーテルの使用は、不適切なデバイス使用の予測因子ではなかった。不適切な血管内デバイスの使用は入院患者に極めて高い頻度でみられる事象であり、デバイス関連の不良な転帰と強く関連している。これらの結果は、血管内デバイスの過剰な使用を組織的に減少させるための対策を確立するうえで有用であると考えられ、これにより有病率、死亡率、および過剰な医療費が減少することが期待される。
