Hand hygiene-related clinical trials reported since 2010: a systematic review

現在、医療従事者の手指衛生遵守を改善することによって医療関連感染を減少させることが極めて重要視されている。また、医療従事者の手指衛生遵守の不良が一般のメディアに認識され、議論されることが増えている。我々の目的は、ピアレビュー後に発表された医療従事者の手指衛生遵守に焦点を当てた研究、特に臨床試験の系統的調査の結果を報告することであった。世界保健機関（WHO）の手指衛生ガイドラインが発表された後の 2009 年 12 月から、PubMed および CINAHL に手指衛生遵守のインデックスが付けられた 2014 年 2 月までに発表された文献を検索した。最初に検索された 57 報の適合性および方法論を検討後、最終的に 16 の臨床試験をレビューの対象とした。大部分は欧米で実施された研究であった。主な研究対象は集中治療室であり、続いて高齢者治療施設であった。最も多く研究対象とされた医療従事者カテゴリーは看護師であり、続いて看護補助者および医師であった。手指衛生遵守報告の分析単位は、「手指衛生のタイミング」であった。4 つの研究は WHO ガイドラインに記載された「私の手指衛生の 5 つのタイミング」の枠組みを採用し、他の論文はデザインの異なる独自の集学的戦略に焦点を当てた。以上より、手指衛生改善の介入戦略に集学的な方法を採用することは、その方法が WHO の枠組みに準拠するか、他の検討された集学的枠組みをに準拠するかによらず手指衛生遵守の中程度の改善をもたらすと考えられる。

監訳者コメント：
WHOの5つのタイミングの与えたインパクトは相当大きく、手指衛生の評価を標準化するためには相当な功績がある。一方で、5つのタイミングを覚えるのには手間取るためより平易な言葉で簡素化しかつ実施率の高まるような仕掛けが必要であろう。
