Relationship between prevalence of device-associated infections and alcohol-based hand-rub consumption: a multi-level approach

多水準ロジスティック回帰モデルを用いて、フランスの医療施設における擦式アルコール製剤の使用量とデバイス関連感染症有病率との関連の有無を評価した。2 種類の全国的データベース、すなわち 2006 年のフランスの院内感染有病率調査、および指標として擦式アルコール製剤の使用量（ICSHA；indicateur de consommation de solution hydro-alcoolique）を含む 2006 年フランス感染制御指標（2006 French Infection Control Indicators）データベースを使用した。医療施設に 2 日以上入院し、1 種類以上の医療器材（尿道カテーテル、血管カテーテル、または気管チューブ）を使用した患者を解析対象とした。多水準統計解析を実施し、患者レベルの変数と病院レベルの変数の共同効果を評価した。対象患者を 15 例以上含む 814 の医療施設を対象とし、合計患者数は 53,459 例であった。デバイス関連感染症の全有病率は 6.7％（95％信頼区間 6.4 ～ 6.9）であった。ICSHA の中央値は 37.2％であった。デバイス関連感染症有病率と ICSHA との間に関連はみられなかったが、患者レベルのすべての変数はデバイス関連感染症有病率と関連していた。デバイス関連感染症有病率の病院レベルでのばらつきの 25％は患者レベルの変数により説明されたが、本モデルに患者レベルの変数と病院レベルの変数の両方を組み入れた場合も、このばらつきの 60％は依然として説明されなかった。デバイス関連感染症有病率と手指衛生との関連をさらに評価するためには、侵襲性医療器材の操作に密接に関連する手指衛生の実践について、研究を重ねる必要がある。
