ドイツ北部のナーシングホーム入居者におけるメチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)の保菌率および分子疫学★

ナーシングホームの入居者は、メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)保菌リスクが高い集団である。感染制御および医療連携の推進を強化するため、ドイツ北部のブラウンシュヴァイクのナーシングホーム入居者における MRSA の時点保菌率および分子疫学を調査した。この地域の 34 施設のナーシングホームのうち 32 施設が研究に参加し、入居者の 68%(2,688 名中 1,827 名)が鼻腔内保菌および/または創部保菌のスクリーニングを受けた。合計 139 名の入居者(7.6%、95%信頼区間 6.4% ~ 8.8%)に MRSA 陽性が確認されたが、これは調査前のナーシングホームの情報から予想された MRSA 保菌者数である 24 名(0.9%)の約 6 倍であった。尿道カテーテル、創傷、入院歴、およびナーシングホームでのケア依存度が高いことなどの既知のリスク因子が特定されたが、MRSA 保菌の単独の決定因子と考えられるような感度の高い因子はなかった。spa タイピングにより、分離株の 70%以上がドイツ北部の病院感染型 MRSA に典型的な Barnim 株(ST-22、EMRSA-15、CC22)に分類されることが示された。市中獲得型または家畜関連の黄色ブドウ球菌株の存在を示すエビデンスはみられなかった。これらのデータは、ドイツ北部では MRSA が病院環境から他の医療施設に拡散しており、現在はこのような医療施設を MRSA 伝播の重要なリザーバであると見なすべきことを示している。

監訳者コメント
本調査では、MRSA 保菌陽性率が最も高い施設では 18%に達している。日本でも長期療養型施設の入所者における多剤耐性菌の保菌は徐々に問題になりつつあり、そういった施設からの転入患者が多剤耐性菌を持ちこむ事例が少なくない。これらの施設における伝播の実態は不明であり、またそれを効果的に制御する手法も明らかでなく、深遠な問題である。
