逆流防止弁は静注輸液の逆流と細菌汚染を防止しない

逆流防止弁は、1 本の静脈アクセスで複数回の注入を行う場合に、輸液が意図した流れ方向(DDF)から逆流することを防止するために設計されている。臨床的条件に相当する遅い注入速度下で、逆流防止弁が DDF からの逆流を確実に防止できるかどうかを in vitro で検討した。汚染輸液の注入に起因するカテーテル関連感染症は患者ケアに関連する問題であるため、逆流防止弁によって近位側(ドリップ側)の輸液の細菌汚染を防止することができるか、また逆流防止弁が医療関連感染の予防に寄与しているかどうかを検討した。さらに、集中治療室(ICU)の点滴装置および輸液の細菌汚染率を測定した。5 種類の逆流防止弁のモデルの 10 個ずつに対して DDF に逆らった低速の流れを in vitro で発生させ、輸液に対する閉鎖性(流入の有無)、およびインジケータである 3 種類の細菌の移入の有無を評価した。次いで、ICU の患者から回収した輸液チューブの汚染状況を調査した。DDF に逆らった圧力を徐々にかけたところ、防止弁のモデルとはほぼ無関係に、評価した逆流防止弁の 40%は輸液の漏出防止ができなかった。30%では細菌が DDF に逆らって移入し、防止弁の近位側から検出された。ICU 患者から回収したチューブサンプルの 6.7%に細菌汚染が認められた。結論として、点滴装置の汚染は ICU のケアに関連する問題である。逆流防止弁では輸液の逆流を確実に防止することはできず、細菌フィルタとしての有用性はない。したがって、医療関連感染を減少させるための手段として逆流防止弁は推奨できない。

監訳者コメント:
構造的な逆流防止弁が、機能的には逆流防止になっていなかったというお粗末な部材特性を露見した論文である。マクロの防止がミクロの(つまり、より高感度な培養法などによる)検証プログラムで調べると乖離することはあり得る。
