医療関連感染症の特定のための新たな方法の検証

管理データを用いて医療関連感染症を特定するこれまでの試みは、大規模解析に使用できる可能性があるにもかかわらず不成功に終わっている。今回の研究では、管理データから得た抗菌薬の使用パターンに基づいて医療関連感染症を特定する新たな方法の精度を検証した。日本の4病院(584例)を対象として、カルテレビュー解析と新方法を同時に独立して実施し、医療関連感染症を特定した。新方法の精度を感度、特異度、陽性的中率、および陰性的中率を用いて定量化し、カルテレビュー解析と比較した。またCohenのκ係数を用いて、特定方法間の一致度を解析した。新方法では感度 0.93(95%信頼区間[CI]0.87 ~ 0.96)、特異度 0.91(95%CI 0.89 ~ 0.94)、陽性的中率 0.75(95%CI 0.68 ~ 0.81)、および陰性的中率 0.98(95%CI 0.96 ~ 0.99)であった。κ係数が 0.78であったことから、両方法の一致度は比較的高いことが示された。今回の結果により、新方法は大規模な患者群を対象とした医療関連感染症の特定に十分な妥当性を有していることが示されたが、陽性的中率が比較的低かったことから、個々の患者レベルでの感染症の特定に使用するには限界があることが示された。この新方法は、大規模な医療関連感染症の特定、疾患費用についての多施設共同試験のリスク補正、または医療関連感染症の制御方法の費用対効果分析の開始時などに適用できると考えられる。
