Routine hand hygiene audit by direct observation: has nemesis arrived?

感染予防・制御の専門家は、医療従事者の手指衛生遵守を直接的観察により監査する手法を開発・実施するために、貴重な保健サービスの時間を費やしてきた。この方法は手指衛生の監査の「ゴールドスタンダード」とされているが、多大な労力を要するものであり、監査プロセスの質管理を確実に実施するために定期的なモニタリングを受けている熟練した職員が実施しない場合は不正確となる可能性がある。「リアルタイム」のデータを得るための新たな工学機器が開発されているが、これらの機器を導入し、定期的な患者ケアの際に使用した場合の費用評価は行われていない。また、手指衛生エピソードや前後の状況については、直接的観察ほどの情報量を得ることができない。手指衛生製品の使用量が直接的観察の安価な代用となる。製品の使用量からは、手指衛生エピソードまたは現場における手指衛生遵守の障壁についての詳細な情報は得られないが、継続的モニタリングのためのツールになり得ると考えられる。現場の問題を特定し実践を改善するために、高度な熟練スタッフが直接的観察による手指衛生実践の直接的評価やスタッフへの聞き取り調査をいつ、どこで実施すべきかは、感染予防・制御チームおよび臨床スタッフがデータを定期的に精査することによって示されるものと考えられる。
