RS ウイルスは早産児の生命を脅かす感染症の病原体となり得る。当院の新生児集中治療室（NICU）で 2010 年 2 月に発生した乳児 4 例のアウトブレイクを報告する。RS ウイルス A 型感染症がリアルタイム PCR で確認された。NICU の全乳児にパリビズマブを投与した。新たな有症状者は認められず、反復的な RS ウイルスサーベイランスにより NICU 内にさらなる交差伝播はないことが確認された。今回のアウトブレイクから、NICU の病床利用率が非常に高い場合は感染制御が困難であること、および分子学的方法はウイルス検出および患児の管理を円滑に進めるうえで有用であることが明らかにされた。

監訳者コメント：
本事例の舞台となった NICU は 14 床で、2 床が個室、残り 12 床が 3 部屋（4 床室）に配置している。日本の大部分の NICU は大部屋であることが多く、本事例よりもさらに劣悪な環境に置かれている。その意味で、RS ウイルスのアウトブレイクのリスクはより高いと考えられる。パリビズマブの治療的および予防的投与は日本でも可能であり、高価な薬剤であるものの、本事例のような場合には費用対効果が高いと考える。それも含め、本事例で経験されたこと、およびその対処方法は大変参考になる。
