アイルランドの病院におけるメチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)およびメチシリン感性黄色ブドウ球菌(meticillin-susceptible S. aureus;MSSA)の死亡診断書による報告

メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)感染症の死亡診断書への記載に関する問題が、社会的に大きな議論を呼んでいる。アイルランドの 3 次紹介病院 5 施設から得たデータを用いて、血液培養により MRSA またはメチシリン感性黄色ブドウ球菌(meticillin-susceptible S. aureus;MSSA)が分離されてから 30 日以内に死亡した院内患者を対象として、死亡診断書に記載された MRSA と MSSA との比較を行った。研究期間中に、死亡前 30 日以内に血液培養により MRSA または MSSA が分離された患者は合計 133 例であった。1 例は死亡診断書の情報を入手できなかったため除外したが、その他の 132 例は本研究に適格であった。血液培養により MRSA が分離された患者は 59 例(44.4%)、MSSA は 74 例(55.6%)であった。1 例は、血液培養から MRSA と MSSA がいずれも分離されたため、両カテゴリーに分類された。MRSA 症例 59 例のうち、死亡診断書に MRSA の記載があった患者は 15 例(25.4%)であった。MSSA 症例 74 例のうち、死亡診断書に MSSA の記載があった患者は 9 例(12.2%)であった。MRSA は MSSA と比較して死亡診断書に記載される割合が高かった(オッズ比 2.46、95%信頼区間 1.01 ~ 6.01、P < 0.05)。これらの結果から、アイルランドでは死亡診断書への病原体や感染症の記載方法は一貫していないと考えられること、および死亡診断書のデータは特定の病原体が関連する死亡率が過小評価されている可能性があることが示唆される。特に、診断書記入者は MSSA よりも MRSA を過度に重視して記載していると考えられる。
