Outbreak of Pantone-Valentine leucocidin-positive meticillin-resistant Staphylococcus aureus in a regional burns unit

地域における熱傷・形成外科病棟において16か月にわたり、単一株のパントン・バレンタイン型ロイコシジン（PVL）産生メチシリン耐性黄色ブドウ球菌（Pantone-Valentine leucocidin-positive meticillin-resistant Staphylococcus aureus；PVL-MRSA）の保菌・感染例30例（患者19例、患者家族1例、職員10例）が認められた。この菌株はシプロフロキサシン、ネオマイシン、およびゲンタマイシンに耐性を示し、英国で蔓延している市中獲得型MRSAの系統に属していた。アウトブレイクは4段階で発生しており、第1段階は熱傷外来の患者、第2～3段階では熱傷病棟、最終段階は形成外科病棟で認められた。保菌・感染例が確認された部門を閉鎖して、蒸気洗浄および過酸化水素処理を含む入念な洗浄を実施したにもかかわらずアウトブレイクは持続した。職員の保菌に対する徹底的な取り組みの後にアウトブレイクの制圧が達成された。

一般的にMRSA対策で最も重要なのは医療従事者による手指衛生と適切な個人防護具（手袋・ガウン・マスク）使用の徹底であり、緑膿菌（Pseudomonas aeruginosa）やアシネトバクター・バウマニー（Acinetobacter baumannii）では加えて環境整備も重要となるとされる。米合衆国やカナダでは毒性の強いクロストリジウム・ディフィシル（Clostridium difficile）の蔓延のために環境消毒に対する関心が高まっているが、基本的な手指衛生や個人防護具使用の徹底がより重要である。技術（technology）よりもそれを使いこなす考え方（concept）が問題である。
