Repeated prevalence surveys of healthcare-associated infections in Finnish neonatal intensive care units

新生児集中治療室（NICU）の5分の1を超える患児が、医療関連感染症に罹患していることが報告されている。本研究の目的は、フィンランドのNICU入室患児の医療関連感染症の有病率、発症の時期、および種類を評価することである。2008年11月から2009年5月に、フィンランドの全NICU（24施設）で月1回の時点有病率調査を6回実施した。調査日に入室中の全患児を対象として、基礎疾患および調査日とそれ以前の6日間に使用した侵襲性デバイスまたは治療薬について、標準化した調査票に記入した。調査日に症状を有するか治療中である医療関連感染症の全患児のデータを記録した。医療関連感染症を早発型（生後72時間以内に発症）または遅発型（生後72時間以後に発症）に分類した。6回の調査で、1,281件の調査票を得た。患児163例に164件の医療関連感染症が認められ（全有病率13％）、このうち63件（38％）が遅発型感染症（有病率6.5％）であった。多くみられた医療関連感染症のタイプは臨床的敗血症、検査確認された血流感染症、結膜炎、および肺炎であった。全医療関連感染症のうち24％が微生物検査により確認された。出生時体重が1,500 g未満の患児は、出生時体重が1,500 g以上の患児と比較して遅発型医療関連感染症の発生頻度が高かった（10％対4％、P ＜ 0.01）。7日以上の入院も、医療関連感染症有病率の上昇と関連していた（8％対3％、P ＝ 0.01）。今回の調査は、参加NICUの医療関連感染症に対する認識の向上に有用であった。これらの結果は、将来的なフィンランドのNICUにおける医療関連感染症発生率のサーベイランスの強化に役立つと考えられる。
