Colonisation by extended-spectrum β-lactamase-producing Klebsiella spp. in a paediatric intensive care unit

小児の基質特異性拡張型β-ラクタマーゼ（ESBL）産生クレブシエラ（Klebsiella）属菌の保菌率と保菌のリスク因子を調べるため、前向きコホート試験を実施した。試験は2008年の5か月間に、ブラジル・ルシフェにある小児集中治療室（PICU）で実施した。PICUへ入室した後24時間以内と2、5、7および14日目に直腸スワブを採取した。Kirby-Bauerディスク拡散法を用いてKlebsiella属菌ESBL産生株の検出を行い、double disc synergy testにより確認した。合計186例の患児を試験に組み入れ、その年齢中央値は3歳であった。ESBL産生Klebsiella属菌の保菌率は14％であり、13例（7％）は入院時にすでに保菌していた。PICU入室における保菌発生率は1,000 患者・日数あたり14.2件であった。多変量解析では、第三世代セファロスポリン系抗菌薬の使用が保菌のリスク因子であった（P ＝ 0.008）。生存解析によって、PICU入室期間が延長すると、累積保菌リスクが増加することが明らかになった。この研究から同様の医療環境における業務改善の指針となるとともに、交差感染の規模と感染制御介入の効果に関する今後の研究の方向性を示すことになる基本的な情報が得られた。

基質特異性拡張型β-ラクタマーゼ（ESBL）産生菌はわが国でも明らかな増加傾向にあり、ESBL産生菌による感染症が増加すれば治療としてカルバペネム系抗菌薬の使用量が増加することになる。諸外国の中にはESBL産生菌が蔓延していると考えざるを得ない状況もあり、わが国においても早期に封じ込める必要があると認識するべきである。
