Yeast contamination of kidney, liver and cardiac preservation solutions before graft: need for standardisation of microbial evaluation

固形臓器摘出の際の保存液の酵母汚染がレシピエントに生命を脅かす合併症を引き起こすことがあり、このような汚染の発生率を明らかにする必要がある。著者らは2004年1月から2008年12月に、当院で回収されたすべての保存液について、保存液10 mLを遠心分離した沈殿を真菌用培地に接種して30℃ 15日間培養とする標準化した手順によって、潜在的な真菌汚染を前向きに調査した。試験期間中に実施された728件の移植手術（腎397件、肝262件、および心69件）から659件（90.5％）の保存液を得た。移植臓器の酵母汚染率は、心0％（62件中0件）、腎3.1％（356件中11件）、および肝4.1％（241件中10件）であった。同定した菌種はカンジダ・アルビカンス（Candida albicans；10件）、カンジダ・グラブラータ（C. glabrata；5件）、カンジダ・クルーゼイ（C. krusei；2件）、カンジダ・トロピカーリス（C. tropicalis；1件）、カンジダ・バリーダ（C. valida；1件）、ピキア・エチェルシー（Pichia etchellsii；1件）、およびロドトルラ（Rhodotorula）属菌（1件）であった。日常的な微生物検査により同定できたのは、これらの真菌汚染21件中5件のみであった。1年以上の追跡後に生存していたレシピエントは20例であり、1例は7か月時点で髄膜癌腫症のために死亡した。3例のレシピエントの術後排液から同一種のCandida属菌が検出されたが、感染症あるいは超音波検査による異常は認められなかった。14例は抗真菌薬投与を受けた。検査した腎・肝保存液全体の3.4％に酵母汚染が認められた。その臨床的な転帰や治療管理については現時点では不明である。この研究から、多くの保存液の分析、真菌用培地への接種、および長時間培養、など、微生物検査法の最適化・標準化が必要であることも示唆される。

監訳者コメント：
わが国においても、2010年7月から施行された改正臓器移植法に伴い、脳死移植の症例数が著増しており、これからは摘出された固形臓器の保存液も感染予防対策の重要な対象となると考える必要がある。
