Post-discharge surgical site infections after uncomplicated elective colorectal surgery: impact and risk factors. The experience of the VINCat Program

背景
大腸手術後の手術部位感染症（SSI）は、医療システムの測定可能な質評価指標である。病院間や国の間での SSI 率の比較が注目されるようになっているが、SSI 発生に関するデータにばらつきがあるため、このような比較は困難である。評価にあたっては、データ収集を標準化し、信頼性の高い退院後サーベイランス（PDS）を行う必要がある。
目的
待機的大腸手術後の退院後 SSI の影響とリスク因子を明らかにすること。
方法
VINCat は、スペイン・カタロニアの院内感染サーベイランスプログラムである。2007 年から 2011 年に 52 病院がこのプログラムに参加した。各病院が、待機的大腸切除術に関する標準化された積極的サーベイランスを前向きに実施した。PDS は集学的アプローチにより実施し、手術後 30 日間は必須とした。
結果
研究期間に実施された待機的大腸手術 13,661 件を対象とした。2,826 例（20.7％）が SSI と診断され、このうち PDS 中に診断されたのは 22.5％であり、その 52％は再入院が必要となった。PDS SSI の患者は入院時 SSI の患者よりも年齢が低く（オッズ比［OR］1.57、95％信頼区間［CI］1.29 ～ 1.91）、女性が多く（OR 1.40、95％CI 1.16 ～ 1.69）、内視鏡手術が多く（OR 1.56、95％CI 1.30 ～ 1.88）、切開部 SSI が多かった（OR 1.88、95％CI 1.54 ～ 2.28）。
結論
VINCat 参加病院の待機的大腸手術での SSI 率は、他の全国的なプログラムの報告と比較すると高値の部類に属する。PDS SSI は SSI の全発生率の増加をもたらし、重大な臨床的影響を及ぼし、SSI の約 4 分の 1 を占めた。低年齢と腹腔鏡手術が最も重要なリスク因子であった。大腸手術のサーベイランスを実施する病院では、標準化された集学的 PDS を行うべきである。

監訳者コメント：
SSI サーベイランスは、在院期間が短い場合は退院後に発生した SSI がカウントされず、見かけ上低く算出される。本論文では手術後 30 日間にわたり、多施設共同で SSI サーベイランスを実施し、その実態を明らかにした。退院後の SSI サーベイランスの必要性を強調している。
