Infection due to C. difficile ribotype 078: first report of cases in the Republic of Ireland

クロストリジウム・ディフィシル（Clostridium difficile）は重要な医療関連病原体である。リボタイプ027などの強毒性株が近年、広く報告されるようになっている。そのほかの強毒性株にはリボタイプ078があり、これによるC. difficile感染症の割合が増加していると考えられている。本報告では、アイルランドの病院で検出された、この病院の管轄区域内のナーシングホーム1施設に発生した15例のリボタイプ078によるC. difficile感染症アウトブレイクについて述べる。リボタイピングを実施した全C. difficile分離株のうち、リボタイプ078は15％を占めていた。リボタイプ078によるC. difficile感染症患者の平均年齢は76歳であった。診断から8週以内に患者の46％に症状の再発が認められ、8件の死亡例中5例でC. difficile感染が直接関与していたと考えられた。DNAフィンガープリント変法を用いて15例のクラスターが同定でき、さらに一部のC. difficile感染症患者間にこれまで認識されていなかった関連性が存在することが示唆された。本アプローチにより、C. difficileの共通の感染源と伝播経路が解明されることが期待できる。
