Disease burden of prosthetic joint infections after hip and knee joint replacement in Finland during 1999窶・004: capture窶途ecapture estimation

1999年から2004年にFinnish Hospital Infection Program（SIRO）で実施された人工関節感染症発生率サーベイランスの評価を行った。Capture窶途ecapture法によりその感度を推計し、人工股関節置換術（THA）および人工肘関節置換術（TKA）後の人工関節感染症発生率を評価した（13,482件）。データソースとしてSIRO、Finnish Arthroplasty Register（FAR）、およびFinnish Patient Insurance Center（FPIC）を使用し、マッチングにより129例の人工関節感染症患者を特定した。SIROの陽性的中率（91％）で補正後の、FARとFPICとの間の交互作用項を含めた対数線形モデルから得られた人工関節感染症発生率推計値は、THA 1.6％（95％信頼区間［CI］1.2％～2.4％）、TKA 1.3％（95％CI 1.1％～1.6％）であった。SIROの感度は36％から57％で、ばらつきがみられた。1年間の人工関節感染症発生率は、THA後は人口100,000人あたり2.1件、TKA後は1.5件であった。人工関節感染症の真の発生率は、全国の定点サーベイランスシステムから通常示唆されている率よりも大きい可能性がある。

監訳者コメント：
待機的かつ無菌的手術を実施する関節置換術においては、感染率が特に重要である。我が国でも国家的なサーベイランスシステムを持つべきであろう。
