Surveillance and endemic vancomycin-resistant enterococci: some success in control is possible

バンコマイシン耐性腸球菌（VRE）は、アイルランドの多くの病院で流行している。VREが流行している施設の2001年から2008年のサーベイランスデータを解析した。臨床的に重要なすべての腸球菌のバンコマイシン感受性を調べた。集中治療室への全入室例を対象として、入室時およびその後は週1回スクリーニングを行った。VRE患者の隔離・コホーティング、月1回のVRE保菌率調査、電子警告システムの導入、手指・環境衛生の改善プログラム、および抗菌薬担当薬剤師との面談などの介入を実施した。VREスクリーニングサンプル陽性数は、2001年（10,000病床日あたりのVREスクリーニング陽性1.96例）から2006年（10,000病床日あたり4.98例）にかけて有意に増加し（P ≦ 0.001）、2007年（10,000病床日あたり3.18例）には減少した（P ≦ 0.01）。VRE血流感染症（BSI）は、2001年の10,000病床日あたり0.09例から2005年の10,000病床日あたり0.78例に増加したが（P ≦ 0.001）、その後は減少した。線形回帰分析から、新規VRE症例数と非隔離VRE患者数との間に有意な関連が認められ、特に2005年5月から2006年12月との間（P ＝ 0.009、95％信頼区間［CI］0.08～0.46）、および2005年5月から2008年12月との間（P ＝ 0.008、95％CI 0.06～0.46）で顕著であった。ルーチンのVREサーベイランスと他の方法と組み合わせることにより、VREが流行している地域や制約のある施設であっても、VRE BSI・保菌を制御することが可能である。

監訳者コメント：
選択圧と保菌圧の両方の改善が感染対策には必要であろう。地域的に蔓延した状態での制御はほとんど困難であり、それ以前の改善が重要である。
