Impact of standardised review of intravenous antibiotic therapy 72 hours after prescription in two internal medicine wards

病院で抗菌薬療法の処方後レビューを実施することは、抗菌薬の適正使用につながり得る。大学病院の2つの内科病棟において、抗菌薬静注療法の処方3日後の標準化レビューの効果を評価した。一方の病棟では、抗菌薬静注療法を受けている患者のカルテを標準化レビュープロセスに従って評価し、処方医にフィードバックした。もう一方の病棟では介入を実施しなかった。6か月後に、介入を行う病棟と非介入の病棟を入れ替えた。介入期間中に全体で204件の抗菌薬投与が対象となり、対照期間では226件が対象となった。処方後レビューにより変更の提案が行われたのは、抗菌薬投与件数の46％であった。処方変更までの期間は、介入期間のほうが対照期間と比較して22％短かった(3.9±5.2日対5.0±6.0日、P ＝ 0.007)。介入群の患者の抗菌薬使用量は対照群と比較して少なかったが、両病棟の抗菌薬総使用量については、今回の介入によって有意な効果は得られなかった。

監訳者コメント：
処方後のレビューを実施しても、実際には治療期間の短縮には至らなかったという結論である。論文作成の意図がよくわからないレポートであるが、処方を助言に基づいて切り替えた症例のほうが、臨床医が単独で投与を判断した症例よりも臨床経過、治療期間などが改善したのかなどを検討するようにデザインすべきであった。
