Building a benchmark through active surveillance of intensive care unit-acquired infections: the Italian network SPIN-UTI

Hospital in Europe Link for Infection Control through Surveillance（HELICS）の集中治療室（ICU）のベンチマークに基づき、定義の標準化、データ収集、および報告手順を確立するために、Italian Study Group of Hospital Hygiene（GISIO-SItI）のItalian Nosocomial Infections Surveillance in Intensive Care Units（SPIN-UTI）プロジェクトを実施した。参加ICUには研究計画の段階からの参加を通じて本プロジェクトの中心的なステークホルダーに加わってもらうため、サーベイランス開始前のミーティングに参集させた。インターネットを介してデータを収集するため、電子データの書式を4種類作成した。サーベイランスについての1か月間のパイロット研究により、本プログラムの全体的な実現可能性の評価、および所要期間と参加病院のリソースの判定を実施した後に、2006年11月から2007年5月までに6か月間の患者対象の前向き調査を行った。SPIN-UTIプロジェクトの対象は、49のICU、2日を超える入院患者3,053例、延べ35,498患者・日であった。感染症の累積発生率は患者100例あたり19.8件、発生頻度は1,000患者・日あたり17.1件であった。最も発生頻度の高い感染症の種類は肺炎であり、感染症に関連した微生物で最も頻度が高かったのは緑膿菌（Pseudomonas aeruginosa）、次いで黄色ブドウ球菌（Staphylococcus aureus）とアシネトバクター・バウマニー（Acinetobacter baumannii）であった。患者のリスク因子を基に層別化した肺炎、血流感染症、中心静脈カテーテル関連血流感染症、および尿路感染症の発生率は、HELICSネットワークのベンチマークとして報告された75パーセンタイル値よりも低かった。SPIN-UTIプロジェクトにより、イタリアの多くの病院で現行のサーベイランスの導入が可能であることが示された。本研究は、リスクに関連する因子を比較するとともに、より深く理解することを目的とした、ベンチマークデータを用いたHELICSプロジェクトに参加する機会を提供できた。
