Feasibility study of a real-time PCR test for meticillin-resistant Staphylococcus aureus in a point of care setting

市販のリアルタイムPCRによるメチシリン耐性黄色ブドウ球菌（Staphylococcus aureus）（MRSA）検査法が、ポイントオブケア検査として利用可能であるかどうかを検討した。GeneXpertシステムをSandwell and West Birmingham病院NHSトラストの4病棟と中央微生物検査室に設置した。GeneXpertシステムは定量的PCR装置であり、これを用いてMRSA検出キットであるXpert MRSAによる検査を実行する。この4病棟に新たに入院した患者の鼻腔内サンプルを双頭スワブで採取した。一方のスワブは検査室以外のスタッフが病棟のXpert MRSAを用いて検査し、もう一方のスワブは微生物検査室で検査した。全体で、患者930例から鼻腔内スワブ988件を採取した（2008年3月から6月）。検査を行ったスワブのうち947件から検査室と各病棟の両方で適切な結果（MRSA陽性または陰性）が得られ、病棟と検査室の結果の一致率は97.5％であった（MRSA陰性850件、MRSA陽性73件）。24件のスワブでは、病棟と検査室で得られた結果が一致しなかった。検査室でのMRSA検査に対する病棟での検査の感度、特異度、陽性的中率、および陰性的中率は、それぞれ83.9％、98.8％、88.0％、98.4％であった。結果が得られるまでの時間（平均）は、病棟のほうが検査室より10時間以上短かった。Xpert MRSA検査は病棟、検査室のいずれで実施しても同様に良好な成績を示した。MRSAに対するポイントオブケア検査の重要な利点は、検査室検査と比べて結果が得られるまでの時間が大幅に短縮されることであった。

Point-of-care-testingとはいわゆる迅速診断のことである。POCTと検査領域では略される。ベッドサイドテストとも呼ばれる。その定義は曖昧だが、おおむね1時間以内に判定可能な検査診断手技と考えてよいだろう。GeneXpertシステムは米国Cepheid社の販売している遺伝子検査システムで、極めて短時間（80分程度）で結果を出すことが可能である。
