Nosocomial swine influenza (H1N1) pneumonia: lessons learned from an illustrative case

2009年春、当施設はニューヨークにおけるブタインフルエンザ（H1N1）パンデミックの「前駆波」の渦中にあった。インフルエンザ様症状を呈する数百人の患者が、A型インフルエンザウイルスの迅速検査を求めて溢れかえっていた。このパンデミックの最中に、急性リンパ性白血病の初発乳児が導入化学療法のために入院した。入院から1週間後、患児は高熱と息切れを呈したが、胸部X線像は正常であった。人工呼吸器を使用するために小児集中治療室（PICU）に移された。パンデミックの最中であったためH1N1肺炎を疑ったところ、逆転写PCR法でH1N1陽性であった。接触者調査から、患児の家族・面会者の中にはインフルエンザ様症状やH1N1を有する者および直近に近接して接触した者はいないことが判明した。またこの調査により、H1N1患児と接触があった小児科医療従事者が、PICUでこの患児のケアを行ったことも明らかになった。個人の特定はできなかったが、小児科病棟とPICUの両方に勤務した医療従事者からH1N1が伝播したと考えられた。本症例は小児科においてH1N1肺炎院内感染が認められた初めての例である。

新型インフルエンザの流行期における施設内感染予防は緊急の課題である。病床運営上、大きな課題だが根本的な解決策はなく、現状で持ち得る対策を徹底するしかない。面会者への対応やスタッフからの発症者等、対策は多岐に及ぶ。
