Thermostable adenylate kinase technology: a new process indicator and its use as a validation tool for the reprocessing of surgical instruments

ルーチンに使用されている自動洗浄消毒器の性能の検証に適した洗浄効果の定量的測定法として、熱安定性酵素アデニル酸キナーゼ（tAK）の評価を行った。市販の洗浄消毒器用ソイルまたは蛋白質を主成分とするソイルを用いて、2種類のインジケータ用材料を調製した。各インジケータは、tAK 100 μgを含むテストソイルをスチールまたはプラスチックの表面で乾燥させたものである。これらのインジケータを洗浄消毒器の各バスケット内に置き、通常の手術で汚染した手術器具とともに洗浄処理した。処理後、携帯用衛生監視機器を用いた迅速酵素アッセイ（検出時間2分）により、tAK残存活性を測定した。自動洗浄消毒器の全サイクル終了後の各インジケータのtAK残量は0.1～0.4 ngであり、除去量の平均log₁₀値は5.8±0.3であった。洗浄消毒器の運転ごとの、または洗浄消毒器内のインジケータの位置によるtAK残存活性に、統計学的な差は認められなかった。tAKインジケータを用いて、洗浄サイクルの各段階における蛋白質除去の分析も行った。その結果、洗浄過程のあらゆる段階が蛋白質除去に寄与しており、主洗浄だけでも蛋白質活性が3.6～4.0 log₁₀低下することが示された。このような迅速な読み取りが可能なインジケータ測定装置を用いた洗浄の定量的指標は、洗浄過程の新しい有効な検証方法となると考えられる。
