Patients’ perspectives on how doctors dress

院内感染は深刻な問題である。感染の拡大への対策として、英国では多くの国民保健サービストラストが「肘以下は露出し、ネクタイは着用しない」とする服装規定を採用している。この規定は、2007年9月に英国保健省が発表した医療従事者の服装に関するガイドラインを受けたものである。病原体が衣服に定着する可能性に関する研究はあるが、服装規定や方針の変更に対する患者の意見に関する研究は行われていない。この問題を検討するため、英国スウィンドンのGreat Western病院の患者75例を対象として質問票調査を実施した。この結果、患者は医師の服装は重要であると考えているものの、ネクタイや白衣の着用は望んでいないことが判明した。また、手術着は好ましい服装であると見なされていた。医師の識別や医師の階級の判別に関する問題の指摘が多かった。患者は概して新しい服装規定を認識しておらず、服装規定と感染制御との関連について知っている患者はほとんどいなかった。今回の調査から、患者は医師に「格式高い」服装を望んでいるわけではないことが示された。医師の識別に関する問題や手術着が適切であるとする患者の意見を考慮すると、「医師の制服」として手術着を導入することを検討すべきである。また、方針変更の告知および感染制御に関する患者の認識向上への取り組みが必要である。

医療従事者に対する固定観念と許容範囲の接点を見いだす研究といえよう。服装の規定は比較的日本のほうが曖昧なのかもしれない。
