英国では病院感染の増加を受けて、政府の保健部門は医療従事者の服装規定を導入した。これらの規定には、半袖の着用、腕時計や宝飾品の禁止、ネクタイや白衣の禁止などの対策が含まれる。これらの用品上には病院病原体が認められるが、これらが病院感染伝播に重要な役割を果たしているとのエビデンスはなく、このような服装規定は多くの批判を受けている。本稿では、新しい服装規定の基盤となっているエビデンスを検討したうえで、最近の規定を正当化する十分なエビデンスはないことを述べる。服装規定は、効果的な感染制御のためというよりも政治的姿勢を表すことが目的のように思われる。また、病院感染を対象とした英国の「ゼロトレランス」令は、機能不全に陥った医療制度と個人の説明責任とのバランスが取れていない。このような服装規定は良好な実践を目的としているのかもしれないが、安価で短期的な解決法への過度な依存は、病院感染の長期的な問題には十分に対応できないと考えられる。

かなり管理者と現場の間の感情論に基づく論文である。対策の骨子がまるで日本の中高生の学生指導のようでもある。装飾品が接触伝播の原因として指摘されているのは従来の論文にあり、エビデンスのあるものないものが混じっている状況である。
