Historical and changing epidemiology of healthcare-associated infections

本総説では、医療関連感染症の歴史的経緯を、現代の変遷する医療関連感染症の疫学像と比較する。疫学像の変遷を裏づけるエビデンスの例として、英国のクロストリジウム・ディフィシル（Clostridium difficile）感染症およびメチシリン耐性黄色ブドウ球菌（Staphylococcus aureus）（MRSA）菌血症による死亡率の推移の変化が挙げられる。また、これらの感染症やこれ以外の医療関連感染症を対象とした現行の国家的介入プログラムの影響について考察するとともに、英国国民保健サービス（NHS）において、医療関連感染症のさらなる減少を図るための公衆衛生戦略の変更に必要な知識のギャップや意見についても言及する。

監訳者コメント：
英国では健康政策（というより公衆衛生政策）の大改革の目玉として、2003年から5年間でMRSAの血流感染症を半減させ、クロストリジウム・ディフィシル感染症についても有意に減少させる目標を国家政策として発表した（Winning ways, working together to reduce healthcare associated infection in England）。この達成のために、上記感染症のサーベイランス実施を法律で義務化し、公衆衛生年報で病院ごとの成績を公表することになった。この努力により2008年までにNHSで運営されているほとんどすべての病院で数値目標を達成した。これは感染制御を現場の努力目標とするのではなく、国民から医療界への具体的な改善成果を客観的数値で要求するという大政策転換であった。この政策は、「医療者の良心だけでは、もはや改善は難しい」という問題点をあぶりだす結果となった。
