Active warming systems to maintain perioperative normothermia in hip replacement surgery: a therapeutic aid or a vector of infection?

大手術（major surgery）の経過で適切な体温を維持するために、信頼性の高い様々な体温調節システムが企画、開発されている。特に高齢患者と衰弱した患者では大きな問題となる、重症化の恐れのある合併症の発症を回避するには、体温調節は不可欠である。このようなシステムの中でも、Bair Huggerブランケットは優れた有効性が実証されている。しかし、一部の報告では、こうした器材の使用によって院内感染症、特に手術創感染症のリスクが増加する可能性が示唆されている。この研究の目的は、人工股関節置換術におけるBair Huggerブランケットの使用と関連した手術部位汚染のリスクを評価することである。そのため、変形性関節症患者に実施したセメントレス股関節インプラント30例で、術中にBair Huggerブランケットを使用した場合と使用しない場合における手術室内の空気細菌汚染レベルを定量化した。非使用中および手術中の手術室で、手術台周辺の種々の区域および患者体表面からサンプルを採取した。結果の統計解析から、Bair Huggerシステムには院内感染の真のリスクはなく、その一方で整形外科の大手術時の低体温がもたらす恐れのある極めて重篤な合併症の予防に有用であることが示された。さらに、術後6か月間の患者のモニタリングにより、その後の院内感染の発生が抑制されることも判明した。

SSI予防の観点から周術期における低体温を防止することの重要性が注目されている。この論文はBair Hugger送風式ブランケットの有用性を示しており、わが国においても外科手術ウォーミングブランケットのさらなる普及が望まれる。
