Reducing surgical site infection incidence through a network: results from the French ISO-RAISIN surveillance system

手術部位感染症（SSI）は院内感染制御プログラムの主要な標的の1つである。著者らは、ISO-RAISIN（infection du site operatoire－Reseau Alerte Investigation Surveillance des Infections）と称する全国的なSSIサーベイランスシステムの8年間にわたる成果を評価した。毎年3か月間、手術を受けた連続症例を登録し、術後30日間調査を行った。標準的な判定基準に従ったSSIの診断や、創分類、米国麻酔科医学会（American Society of Anesthesiologists；ASA）スコア、手術時間、待期的手術/緊急手術、および手術の種類などのリスク因子に関する個々の患者のデータを、標準化した書式に記入した。1999年から2006年の間に838の参加病院で手術を受けた964,128例に、14,845件のSSIが確認された（全体の粗発生率1.54％）。全体のSSI粗発生率は2.04％から1.26％に（P＜0.001、相対減少率－38％）、全米病院感染サーベイランスシステム（NNIS）-0で補正したSSI発生率（監訳者註：SSIサーベイランスにおけるNNISリスクインデックスが0の症例のみのSSI発生率）は1.10％から0.74％に（P＜0.001、相対減少率－33％）低下した。顕著なSSI発生率低下が観察されたのはヘルニア修復と帝王切開であり、次いで胆嚢摘出術、人工股関節形成術、および乳房切除術であった。病院ネットワークを利用したベンチマーク達成を目指す積極的サーベイランスは、SSI発生率を低下させる有効な戦略である。
