Ten-year air sample analysis of Aspergillus prevalence in a university hospital

1995年から2005年までの10年間、三次ケア大学病院で空中浮遊真菌サンプルを毎月採取した。ペア・サンプルを25℃と37℃で培養した。各場所に設置されている空気ろ過システムごとにデータを解析した。患者ケア区域で採取して37℃で培養したサンプルの平均回収真菌数は、外気サンプルの平均回収真菌数の18％であった（22 対 122 cfu/m3）。HEPAフィルタが設置されている場所で採取して37℃で培養したサンプルでは、約1/3がアスペルギルス属菌陽性であった。その他の患者ケア区域では1/2が陽性、外気サンプルは95％が陽性であった。37℃の培養で平均値＋3 SDを超える真菌数となる散発的なバーストは48件認められた。院内感染発生とAspergillus fumigatus回収数高値との関連は、少なくとも1カ所で認められた。非実用的な方法を用いなければ、アスペルギルス属菌を完全に排除した環境を提供することは不可能である。著者らは、ルーチンの空気サンプル採取は、院内感染を予測する有効な方法ではないと結論する。しかし、一過性のスパイクまたはバーストは、院内の汚染源を特定するうえで有用であると考えられ、またリスク患者に対する介入治療の追加を検討する際に利用できる。重視すべきは、外気の高性能ろ過を継続すること、およびその他の環境制御方法により環境中の日和見真菌胞子の拡散防止を確実に図ることである。

監訳者コメント：
やはり定期的な環境調査から感染管理の実践に繋げるのは難しく、ルーチン業務での環境調査はムダであると考えるべきであろう。新しく2007年6月に改訂された米国の隔離予防策のガイドラインでも、このような空調管理までも含めた防御的環境を提供すべきなのはグラフト生着前の造血幹細胞移植患者に限定して推奨されている。私たちもマンガ「もやしもん」（原作：石川雅之、イブニング連載中（講談社刊）、一部地域で深夜時間帯にアニメーションも放映中）の主人公みたいに菌が見えるとよいのだが。
