A review of dental implants and infection

歯科インプラントが歯牙欠損の管理に一般に用いられるようになっている。インプラントは汚染された手術野に埋入されるものの、その成功率は比較的高い。本稿では歯科インプラントに関するレビューを行い、感染およびインプラントの失敗をもたらす因子に焦点をあてる。文献検索を実施し、インプラント周囲の感染における微生物組成について分析した研究を特定した。歯科インプラント周囲炎の微生物叢は慢性歯周炎に類似しており、ポルフィロモナス・ジンジバリス（Porphyromonas gingivalis）やプレボテーラ・インターメディア（Prevotella intermedia）などの嫌気性グラム陰性桿菌、ベイロネラ（Veillonella）属菌などの嫌気性グラム陰性球菌、トレポネーラ・デンティコーラ（Treponema denticola）などのスピロヘータが優勢であった。整形外科での感染症によくみられる黄色ブドウ球菌（Staphylococcus aureus）とコアグラーゼ陰性ブドウ球菌の意義については異論があるが、臨床的感染部位から分離された場合には意義を有していることは明らかである。また、大腸菌群とカンジダ（Candida）属菌の病原体としての関与についても、さらなる縦断的研究を実施する必要がある。現時点では、歯科インプラント埋入に対する標準的な予防的抗菌薬レジメンや、インプラント周囲炎に対して広く受け入れられている治療法はない。感染が生じたインプラントの治療は困難であり、通常は抜去が必要となる。英国には、インプラント施術後の系統的なサーベイランスプログラムは存在しない。このため、そのようなプロジェクトを策定することが望ましく、それによって有用な疫学的データが得られると考えられる。
