Reduction in surgical site infection in patients treated with microbial sealant prior to coronary artery bypass graft surgery: a case-control study

心臓手術後の手術部位感染症（SSI）は重篤な合併症の1つである。今回の症例対照研究では、冠動脈バイパス（CABG）術施行患者を対象として、シアノアクリル酸ベースの皮膚用微生物シーラント（InteguSeal^R）術前塗布がSSI率に及ぼす影響を調査した。2007年3月から11月に併用処置の有無にかかわりなくCABG術を受けた患者676例のうち、545例に標準的術前ケアを行い、131例には微生物シーラントによる前処置を併用した。微生物シーラントによる前処置を受けた131例中90例と対照90例とを、SSIリスク因子であることが確立している術前および術中の因子でマッチさせた。術前のSSIリスクスコアは微生物シーラント群9.9±4.3、対照群9.7±4.0（P＝0.747）、術前から術中のリスクスコアはそれぞれ9.7±4.1、8.7±3.5（P＝0.080）であった。微生物シーラント群では頸動脈病変（P＝0.019）、うっ血性心不全（P＝0.019）、急性心筋梗塞（P＝0.001）、および緊急手術（P＝0.026）の頻度が有意に高かった。追跡率は両群とも100％であった。術後30日間に浅部または深部の胸骨創感染を発症した患者は対照群7例（7.8％）、微生物シーラント群1例（1.1％）であった（オッズ比7.5）。まとめとして、術前患者への前処置に微生物シーラントを追加することにより、CABG術後のSSI発生率が低下すると考えられるが、明確な結論を導く前にさらなる大規模試験を実施する必要がある。

監訳者コメント：
術前に皮膚に対して微生物シーラントの処置を行ってSSIを減少させたという興味深い研究である。症例対照研究であるため、処置群（症例群）と無処置群（対照群）の背景因子に差がでているが、処置群のほうがむしろ術前合併症などの背景因子は悪く、にもかかわらずSSIは対照群のほうが多かった。微生物シーラントのSSI防止効果、特に心臓血管外科や整形外科などの清潔手術における有用性を強く示唆する論文である。
