Molecular characterisation of outbreak-related strains of vancomycin-resistant Enterococcus
faecium from an intensive care unit in Beijing, China

2006年9月から2007年8月に20床の集中治療室（ICU）で14例の患者に発生したバンコマイシン耐性エンテロコッカス・フェシウム（Enterococcus faecium）によるアウトブレイクを調査した（発生率は1,000 ICU患者日あたり3.56例）。vanA型E. faeciumの分離株18株をパルスフィールド・ゲル電気泳動により解析したところ、合計14タイプが検出された。Multilocus sequence typing（MLST）法により、異なる8つの配列型（ST）（ST78、ST117、ST203、ST316、ST362、ST363、ST364、ST365）が同定され、このうち4つ（ST362、ST363、ST364、ST365）は新型であった。17株はclonal complex CC17に属しており、このうち16株はesp遺伝子を保有していた。vanA型バンコマイシン耐性腸球菌をコードする18個のTn1546様配列は3つのタイプ（1型～3型）に分類され、これらはすべてIS1216VとIS1542の両方の挿入配列を保有していた。フィルターメイティング法（filter mating method）により、vanA型E. faecium分離株14株のバンコマイシン耐性は、1.3×10^－6～6.4×10^－5の頻度でE. faecium間で伝達されていることが示された。今回の結果は、本ICUのアウトブレイク中にCC17 E. faecium間でTn1546様配列の接合伝達が生じたことを示唆している。

監訳者コメント：
10^－6から10^－5の頻度で接合伝達が発生するということは、微生物が一晩で10⁶ cfu以上まで増殖可能であることを考えると、耐性遺伝子が菌株間で非常に高頻度に伝達されていることがわかる。しかもひとたび耐性遺伝子を受け取ると、感受性株が一気に耐性株になるということなので、①分裂増殖（垂直方向）と、②遺伝子の伝播（水平方向）の両方向で耐性株が急速に広がる特性があり、コントロールは非常に困難である。これに対しMRSAでは、基本的に②が主方向なので、手指衛生と接触感染予防策で十分コントロールが可能である。こういったことから、MRSAがコントロールできていない施設では、VREが持ち込まれるとあっという間にアウトブレイクしてしまうので、注意が必要である。
