Role of contaminated aspiration tubes in nosocomial outbreak of Klebsiella pneumonia producing SHV-2 and CTX-M-15 extended-spectrum β-lactamases

マダガスカルのアンタナナリボ市にある2つの病院に2006年2月から3月に入院した新生児10例から、セフタジジム耐性肺炎桿菌（Klebsiella pneumonia）が分離された。主な環境感染源は、特に1つの病院では、小児病棟で吸引管を洗浄するために使用される液体であった。アンタナナリボの病院では、管の洗浄に使用する水道水の定期的な交換が行われないため、吸引管による汚染リスクが極めて高い。全臨床分離株の表現型解析（生物型分類と抗菌薬感受性分類）および遺伝子型解析（パルスフィールド・ゲル電気泳動）により、9症例は単一クローンに起因することが示された。このクローンは、SHV-2およびCTX-M-15 β-ラクタマーゼをコードする遺伝子を保有していた。本稿は、マダガスカルの病院での器質特異性拡張型β-ラクタマーゼ産生腸内細菌科最近の流行に関する最初の報告である。

器質特異性拡張型β-ラクタマーゼ（ESBL）産生菌は世界的に問題になっている。わが国でもプロテウス・ミラビリス等の検出頻度が高くなってきており、注意が必要である。
