Nosocomial infection surveillance and control: current situation in Spanish hospitals

2006年のスペインの病院における医療関連感染サーベイランスと制御プログラム、利用されている人的・物的資源を調査した。2005年全国病院目録に登録されているすべての病院に、医療関連感染サーベイランスと制御のための構成、組織、資源に関する質問票を送付した。合計237病院（29.8％）から回答を得たが、これは登録された病床数では55.9％に相当する。約92％の病院がサーベイランスと制御のプログラムを有していたが、29.9％の病院ではその期間は5年未満であった。感染制御看護師数が250床あたり1名を満たす病院は17.4％のみ、感染制御医師数が500床あたり1名を満たす病院は36.2％のみであった。サーベイランスと制御を担当するこれらの看護師・医師の主要な業務はサーベイランスであり、実施率が高い手法は微生物学的検査結果のレビュー、アウトブレイク調査、有病率調査、特定の手技および集中治療領域における発生率調査であった。多施設研究への協力にはばらつきがみられた。スペイン院内感染有病率調査EPINEは医療施設の80.5％で毎年実施されている。重症患者感染発生率研究ENVIN-UCIは24.8％、欧州外科手術感染発生率研究HELICSは22.7％で実施されている。標準的なサーベイランスシステムおよび制御システムを確立するために、専門家の人的資源を改善することを優先すべきである。

より実効的な感染予防管理プログラムのためには、人的および物的な資源を確保するための予算が必要であり、安全はタダでないということを共通認識とする必要がある。
