カンジダ(Candida)属菌は重要な医療関連病原体であり、予防・制御戦略には感染源の特定が重要である。本研究の目的は、カンジダ血症の感染源の可能性としてのカンジダ保菌部位の評価である。カンジダ血液培養陽性の連続症例63例を対象とした。監視培養用の検体を尿、直腸、口腔咽頭、皮膚、血管内カテーテル先端、およびカテーテル周辺の皮膚から採取した。患者の血液と培養部位から分離されたカンジダ種が同一の場合は、分子タイピングを行った。カンジダ血症に関連した菌はカンジダ・アルビカンス(C. albicans)42%、カンジダ・パラプシローシス(C. parapsilosis)33%、カンジダ・トロピカリス(C. tropicalis)16%、カンジダ・ギリエルモンディ(C. guilliermondii)、カンジダ・クルーセイ(C. krusei)、カンジダ・グラブラータ(C. glabrata)、カンジダ・ホルミイ(C. holmii)、およびカンジダ・メタプシローシス(C. metapsilosis)がそれぞれ2%であった。カテーテル先端から分離されたC. parapsilosis10株中6株は、同一患者の血液由来分離株と区別できなかった(すべて新生児)。血液由来C. albicans分離株は、26例中13例では消化管の分離株と、また2例ではカテーテル先端の分離株と区別できなかった。この結果から、C. albicansによるカンジダ血症の感染源として疑われるのは消化管保菌であり、C. parapsilosisによるカンジダ血症の感染源は外部保菌であることが示唆される。

監訳者コメント:
近年、安価で簡便なカンジダ属菌の同定培地が汎用されるようになり、この領域の研究が進んでいる。一方でカンジダは高度な微生物なので、型別・薬剤感受性試験の安定性は一般細菌のようにはいかず、解釈には注意が必要である。
