長期ケア施設における胃腸炎のアウトブレイクについて、閉鎖集団[96床、発症率41%、R0(基本感染拡大率)3.74、感染拡大時間約1日、罹病期間約2日、入院期間は理論的に無限(1,000日)]における感染動態のSEIR[感受性集団(Susceptible)、曝露/潜伏期集団(Exposed/Latent phase)、感染/有感染性集団(Infected/Infectious)、回復集団(Recovered)]コンパートメントモデルを用いて分析した。患者の入退院の変動をシミュレーションして、入院期間が開放集団において胃腸炎の流行期が持続するだけの流行レベルに及ぼす影響を検討した。その他のパラメータをすべて一定と仮定して、入院期間が0.1日(通院治療)から1~2日へ延長すると、流行期(アウトブレイク発生後50日間)の有病者数は5例から18例へと明らかに増加した。有病者数は入院期間の延長とともに急激に減少して、入院期間が50日を超えると5例未満となった。結論として、ノロウイルス胃腸炎アウトブレイクによる有病者数は、病棟の患者の入退院数に大きく依存する。通常の入院期間(0.1~20日)では、有病者数の水準が高くなり、施設の入所患者集団における胃腸炎の流行が持続する。入院期間が20日を超える長期療養施設では患者の入退院数が少なく、流行が持続せずアウトブレイクが自然消滅すると予想される。急性期ケア病院でアウトブレイクが発生した場合、感染伝播の連鎖を断つため、病棟閉鎖などの感染制御対策の強化が必要である。

前掲のBeersmaら、Kanervaらの論文に対する監訳者コメントを参照。
この論文で示された数理モデルによれば、急性期ケア病棟であれば、そんなややこしい感染対策よりも病棟閉鎖のほうが有効であると結論されてしまっており、現実的なような厳しいような複雑な感想をもたなくもない。
