Interventional evaluation of environmental contamination by vancomycin-resistant enterococci: failure of personnel, product, or procedure?

洗浄剤、清掃手順、または清掃従業員の作業能力が変わることによって、環境汚染除去の改善を効率的に達成できるかどうかは不明である。バンコマイシン耐性腸球菌（VRE）による環境汚染に対する清掃作業能力の影響を評価するため、内科集中治療室と呼吸器回復期病棟に環境清掃の多面的改善介入を導入し、連続的な試験を実施した。介入は清掃従業員に対する教育的講義および清掃従業員の行動観察プログラムで構成され、洗浄剤や手順書の変更は行わなかった。介入実施後には、環境清掃率は49％から85％（P＜0.001）に改善し、環境汚染率は、清掃前では21％から8％（P＜0.0001）に、清掃後では13％から8％（P＜0.0001）に低下した。清掃率と汚染率の改善は、ウォッシュアウト期間も持続していた。多変量モデルでは、清掃の徹底が環境汚染の程度に強く影響することが示され、環境清掃率が10％上昇するごとにVRE存在率が6％低下した。これらの結果から、VREによる表面汚染は清掃手順や洗浄剤によるものではなく、清掃の不徹底によることが示唆される。

監訳者コメント：
清掃の徹底が環境清浄化に必要であり、清掃を徹底するために教育と行動観察が必要である、という趣旨の論文だが、後半部分は清掃に限らず手指衛生などに共通する、感染管理の基本ともいえることがらであろう。教育のみではダメであり、行動観察により実行されていることを確かめる点が最も重要であると考える。
