Descriptive study of selected healthcare-associated infections using national data 1996-2006 and comparison with mandatory reporting systems

1996年から2006年の英国イングランドのHospital Episode Statistics（HES）のデータを用いて、クロストリジウム・ディフィシル（Clostridium difficile）の記録を65歳以上の入院患者と4種類のいずれかの整形外科手術を受けた全患者と比較するために記述的研究を実施した。その結果、C. difficile感染率は上昇したが、整形外科手術による手術部位感染発生率は低下したことが示された。両患者の感染はいずれも、高齢女性および重度の併存疾患を有する患者に多くみられたが、地域間の感染率の差はほとんど認められず、社会経済状態のスコアにもばらつきがあった。2004年と2005年について、HESのデータを保健保護局（Health Protection Agency）の義務的報告システムのデータと比較した。その結果、C. difficile感染の記録は、HESのデータより健康保護局のデータのほうが多かった。対照的に、整形外科手術による手術部位感染については、4種類の整形外科手術すべてにおいて、HESのデータには保健保護局のデータよりも多くの手術部位感染と手術件数が記録されていたが、感染率自体はほぼ同等であった。これらの結果は、いずれの方法にも限界があることを反映しており、著者らは、このような重大な医療関連感染についてより完全な実像を把握するために、両方の情報源を個別に使用する、または個人のレベルで相互に関連づけて使用する可能性を提案したい。より優れたコード方法が進捗するか、HESのデータで優れたコード方法の使用が義務化されれば、整形外科手術による手術部位感染の有効なサーベイランスのために、現行の二重の記録システムは不要となるであろう。
