再発性クロストリジウム・ディフィシル(Clostridium difficile)感染症のリスク因子の評価のためのメタアナリシス★

クロストリジウム・ディフィシル(Clostridium difficile)感染症は、病院感染下痢症の原因として最も頻度が高い。患者の15~20%にC. difficile感染症が再発すると推計されている。C. difficile感染症再発のリスク因子に注目した研究はわずかである。C. difficile感染症再発のリスク因子を評価するために、観察研究および無作為化対照試験を対象としたメタアナリシスを実施した。PubMedデータベースを使用して、検索語「C. difficile関連下痢症(Clostridium difficile associated diarrhoea)」または「偽膜性大腸炎(pseudomembranous colitis)」により研究を特定した。観察研究と無作為化対照試験の両方を対象とした。合計1,215件の試験が特定され、このうち48件が採用基準に合致した。C. difficile感染患者1,382例を対象とした12件の試験が、適格性の要件をすべて満たした。オッズ比および研究の質に関する情報を、2名の研究者が個別に抽出した。3件以上の試験で評価されているリスク因子を解析対象とした。C. difficile感染症再発リスク増加と有意な関連が認められた因子は、C. difficile感染症診断後のC. difficileを標的としない抗菌薬の継続投与(OR 4.23、95%CI 2.10~8.55、P<0.001)、制酸薬の同時併用(OR 2.15、95%CI 1.13~4.08、P=0.019)、および高齢(OR 1.62、95%CI 1.11~2.36、P=0.0012)であった。有意なリスク因子をC. difficile感染症再発のリスク因子と見なした。C. difficile感染症再発を予防するためには、これらの患者に対して介入を追加または新規に実施する必要があると考えられる。

わが国ではいまだ大きなアウトブレイクの事例が少なく、CD毒素陽性の株も少ないが、いずれ問題になるであろうから、注目しておくべきである。
