手術室のコンピューターキーボード汚染への麻酔医の関与★★

Anaesthetists’ role in computer keyboard contamination in an operating room

麻酔医が、麻酔処置の記録をコンピューターに保存するときには、通常は汚れて濡れた手袋を装着したままでデータ入力をしている。手術室のコンピューターの汚染、または麻酔医による手洗いや手指消毒の重要性の認識についての研究は行われていない。我々は、キーボード汚染の4つの要素、すなわち(1)汚染の程度、(2)エタノールによる清掃の効果、(3)キーを叩くことによる手袋とキーボードの間の細菌の伝播、および(4)麻酔医の手指消毒の頻度について調査した。キーボード上の細菌の多くはコアグラーゼ陰性ブドウ球菌およびバチルス(Bacillus)属であったが、メチシリン耐性黄色ブドウ球菌も検出された。エタノールによるキーボードの清掃により細菌数は効果的に減少した。濡れた汚染手袋とキーボードの間のメチシリン感受性表皮ブドウ球菌(Staphylococcus epidermidis)伝播は、乾燥した汚染手袋とキーボード間よりも容易であった。手指消毒を行った麻酔医は、麻酔処置後は64%、昼食前は69%であったが、麻酔処置前では17%のみであった。交差汚染を防止するためには、製造者の取り扱い説明書に従いキーボードを定期的に掃除し、1日1回、または血液や分泌物で肉眼的に汚染された場合には消毒を実施する必要がある。さらに麻酔医は、手術室における医療関連感染の原因菌を、自身が伝播させる可能性があることを認識する必要がある。麻酔医は麻酔処置の前後に手指消毒を行い、各処置後にコンピューターを使用する前には手袋を外す必要がある。

東京女子医科大学からの報告で、この状況は日本全国の多くの病院に当てはまるのではないでしょうか。私も読みながら、大きくうなずいてしまいました。今までは「外国のデータだから」といって取り合わなかった麻酔科の責任者に見せても、今度は「これは女子医のデータだから、うちとは違います」と言われるかも知れませんね。
