An outbreak of Serratia marcescens septicaemia in neonatal intensive care unit in Gaza City, Palestine

2005年1月から12月にかけて、ガザ地区にある国営病院の新生児集中治療室でセラチア・マルセセンス（Serratia marcescens）感染症の症例対照研究を実施した。院内感染による敗血症であることが確認された159例の新生児の血中からS. marcescensが検出され、このうち70例（44％）がS. marcescens感染症により死亡し、89例が回復した。主な臨床症状は低体温（38％）、黄疸（42％）であり、29％が1分後のアプガースコア4点、5％が5分後のアプガースコア5点であった。S. marcescens感染と有意に関連する危険因子は、出生時体重1,500 g未満（OR 1.7、P＝0.026）、在胎期間37週未満（OR 2.0、P＝0.002）、および人工呼吸器の使用（OR 2.3、P＝0.001）であった。寒天拡散法による感受性検査から、S. marcescensは全般的にイミペネム感性であり、次いでシプロフロキサシンとオフロキサシンに感性であることが示された。著者らは、新生児の敗血症の発症と関連する可能性がある危険因子を同定したが、重篤な感染症を予防するためには適切な感染制御対策が重要であることを強調したい。

要約には書かれていないが、本事例ではタイトルにあるようにアウトブレイク事例の報告でもある。2005年4月～6月の間に100例近いS. marcescens検出患者が集中しており、その後減少しているが、3か月もの間に月30例以上の患者が発生している状況は決して好ましくなく、集団発生対応としては標準的とはいえない。一方で、これだけ多くのS. marcescens検出症例が集まることも少なく、その意味では貴重な報告ともいえる。S. marcescens感染症のリスク因子として挙がってきたものの多くは、介入が困難なものであり、その意味でも手指衛生や接触予防策などの基本的な感染対策が重要といえる。
