近年、多くの先進国でクロストリジウム・ディフィシル(Clostridium difficile)関連下痢症(CDAD)の症例が増加している。広域スペクトル抗菌薬のリスクに対する認識が高まり、抗菌薬の使用が全体的に減少し、病院衛生がさらに重視されているにもかかわらず、このような事態が生じている。症例の増加の一部は新規強毒株の登場に起因するが、症例の増加は強毒株の報告に先立っている。この流行はプロトンポンプ阻害薬(PPI)の使用の増加と時期的に一致しており、英国や他国のガイドラインに照らすと、その使用の多くは不適切である。胃酸は消化器感染に対する主要な宿主防御の役割を担っており、疫学研究および動物実験から、CDAD発生率とPPIの使用との間には正の関連があることが示されている。PPIの使用は、CDADに対する初回治療成功後の再発と関連することも明らかにされている。C. difficileの栄養型は通常の胃内pHで速やかに殺菌されるが、PPI服用患者の胃内pHでは生存する。栄養型は、湿潤な表面上では患者間の伝播発生に十分な期間、生存することが、最近示されている。結論として、PPIの使用を適切な適応症を有する患者に限定することは、これらの薬剤の不必要な出費の削減につながるとともに、現在のCDADの流行を制御する新たな手段になると考えられる。

監訳者コメント:
治療のための投薬が、CDADを引き起こす契機を生み出す可能性について示唆した論文である。CDADの発症には多くの因子が絡むが、過剰な投薬を避けることでCDADのリスクを少しでも回避できるのであれば、積極的に取り組むべきである。
