抗菌薬関連のクロストリジウム・ディフィシル(Clostridium difficile)下痢症により、大腿骨近位部骨折手術からの回復が困難になることがある。著者らの診療科で実施した抗菌薬の予防投与法の変更を評価するために、4年間の症例対照研究を実施した。2003年1月から2005年1月までの期間は、患者にセフロキシム(1.5 g)の予防投与を3回実施した。その後、麻酔導入時にセフロキシム(1.5 g)とゲンタマイシン(240 mg)を単回投与する新規レジメンを導入した。予防投与の変更前には912例が大腿骨頸部骨折の手術を受け、2005年3月から2007年3月には899例が新規レジメンによる手術を受けた。C. difficile感染が発生したのは、変更前の患者群では38例(4.2%)であったのに対し、新規レジメン群では14例(1.6%)であった(P=0.009)。この期間中に同病院の他科ではC. difficile感染の発生率は上昇した。C. difficile感染患者では、抗菌薬投与日数、入院期間、合併症発生回数、および入院死亡率が統計学的に有意に増加した。抗菌薬予防投与法の選択に関する主な課題は、メチシリン耐性黄色ブドウ球菌(MRSA)による感染症とC. difficile関連下痢症である。この高リスク患者群のC. difficile感染症を予防するためには、セファロスポリン系抗菌薬反復投与の代替法として、この新規レジメンの使用を推奨する。

監訳者コメント:
セフロキシムナトリウム(cefuroxime sodium[CXM];ジナセフ[Zinacef])を採用することにより、周術期のC. difficile感染を減少させたとする論文である。C. difficile感染以外の感染症の発生割合など、検討すべき課題は多いが、常在菌叢を乱すことなく目的菌にターゲットした殺菌が可能であれば取り組むに値する試みである。
