Four Country Healthcare Associated Infection Prevalence Survey 2006: overview of the results

医療関連感染有病率を推計するため、2006年2月から5月にかけてイングランド、ウェールズ、北アイルランド、およびアイルランド共和国の急性期ケア病院で成人患者の調査を実施した。合計75,694例の患者を調査したところ、このうち5,743例に医療関連感染が認められ、有病率は7.59％であった（95％信頼区間7.40％～7.78％）。医療関連感染有病率は、イングランドで8.19％、ウェールズは6.35％、北アイルランド5.43％、アイルランド共和国4.89％であった。頻度が高い医療関連の各臓器感染症は、消化管系（全医療関連感染の20.6％）、尿路系（19.9％）、手術部位（14.5％）、肺炎（14.1％）、皮膚・軟部組織（10.4％）、原発性血流感染（7.0％）であった。MRSA保有率は1.15％であり、MRSAが原因菌であった割合は全器官系感染の中で15.8％であった。クロストリジウム･ディフィシル（Clostridium difficile）有病率は1.21％であった。本調査は、この4か国でこれまでに実施された最大規模の医療関連感染有病率調査であった。使用した方法および組織は、今後の医療関連感染調査イニチアチブにおける国、地域、または部門レベルでの基本的な枠組みとなる。本調査から得られた情報は、医療関連感染の減少を図るための継続的な取り組みにおいて、資源の優先順位を決定する上で寄与するとともに、保健省、病院、およびその他関連団体とって有用なものである。

監訳者コメント：
連合王国（UK）を中心に270病院が参加した大規模調査であり、従来から報告されていたように、急性期ケア病院では5％から10％の患者が何らかの医療関連感染を経験する。この論文も同様の結果を報告しているが、この調査は英国（イングランド）保健省が英国病院感染学会（HIS；Hospital Infection Society）に依頼して実施されたものである点からも大変に興味深い。
