Hospital Infection Society Prevalence Survey of Healthcare Associated Infection 2006: comparison of results between Northern Ireland and the Republic of Ireland

英国とアイルランドを対象とした第3回医療関連感染有病率調査の一環として、医療関連感染の点有病率調査が北アイルランドおよびアイルランド共和国で実施された。本稿では、医療制度が異なる2か国で、類似の方法および同一の医療関連感染の定義を用いた調査結果を比較することの有益性を検討する。アイルランド共和国では44、北アイルランドでは15の成人急性期病院が参加し、合計11,185例の患者（北アイルランド3,644例、アイルランド共和国7,541例）が調査対象となった。全医療関連感染有病率は北アイルランド5.4％、アイルランド共和国4.9％であった。医療関連感染、器材関連の医療関連感染、または血流感染を伴う医療関連感染の各有病率は2か国間で有意差はみられなかったが、メチシリン耐性黄色ブドウ球菌による医療関連感染の有病率には相違が認められた（P＝0.02）。北アイルランドでは尿路感染およびクロストリジウム・ディフィシル（Clostridium difficile）感染が有意に多かった（それぞれP＝0.002、P＜0.001）。両国とも、65歳を超す患者および集中治療室入室患者の医療関連感染有病率が高かった。また、人工呼吸器装着患者、インプラント手術以外の手術直後の患者（アイルランド共和国）、または医療関連感染の危険因子が多い患者の医療関連感染有病率が高かった。北アイルランドとアイルランド共和国の医療関連感染有病率の比較が直接行われたのは、今回が初めてであった。両国の医療関連感染有病率の類似点と相違点を綿密に検討することにより、その結果が医療計画に影響を与えると同時に、公衆に対して医療関連感染の重要性および医療関連感染対策が実施されていることを認識させることが期待される。
