Pertussis outbreak on a neonatal unit: identification of a healthcare worker as the likely source

新生児室における百日咳アウトブレイクの調査および制御について報告する。疑わしい症例に対しては、細菌培養、PCR法、および血清学的検査により確定診断を行った。新生児2例が百日咳症例であることを特定し、遷延性咳嗽を有する看護師を感染源の疑いがあるとして追跡した。実施した感染制御上の介入は、医療従事者および患者に対する予防的な集団化学療法、および咳嗽を有する医療従事者の勤務からの除外などであった。PCR法の使用により、アウトブレイクの程度の迅速評価が可能となった。今回のアウトブレイクにより、若年成人における百日咳菌伝播が引き起こす入院中の新生児へのリスクが明らかになり、またアウトブレイク程度の迅速評価に対するPCR法の有用性が示された。英国では、すべての青少年に対するワクチン接種または医療従事者に対する選択的ワクチン接種などの予防戦略を考慮すべきである。

監訳者コメント：
百日咳は日本でも昨年から今年にかけて注目されている感染症である。市中の人々に対する公衆衛生的施策（ある年齢層に対する普遍的ワクチン接種など）はともかく、医療従事者は麻疹や風疹同様に百日咳に対する免疫状態を確認するか、ワクチン接種を行う時期に来ているのかもしれない。診断法としてのPCR検査にも注目である。
