Prevalence of anti-legionella antibodies among Italian hospital workers

本研究では、長期にわたりレジオネラ汚染を認める病院の医療従事者の抗レジオネラ抗体保有率を評価した。病院はイタリア北部のミラノとトリノ、およびイタリア南部のナポリとバーリに位置する。医療従事者、医歯学生、および血液ドナーについて抗体保有率および抗体力価を評価した。全体で被験者の28.5％が抗体陽性であり、最も検出頻度が高かったのはレジオネラ・ニューモフィラ（Legionella pneumophila）血清群7～14であった。被験者の地域が異なると、レジオネラ抗体の血清保有率および血清型に大きな差が認められた。ミラノでは医療従事者の抗体保有率が血液ドナーよりも有意に高かったが（35.4％対15.9％、P＜0.001）、ナポリでは両群とも高い抗体保有率を示し（48.8％対44.0％）、L. pneumophila血清群1～6の抗体の割合が高かった。バーリでは歯科従事者のほうが事務職員よりも血清陽性率が高かったが、歯科部門の給水装置の汚染を避けるために毎日消毒処置を実施しているトリノではそのような傾向はなかった。抗体保有とレジオネラ症の危険因子の存在との間に関連は認められず、肺炎および/またはインフルエンザ様症状との間にも関連は認められなかった。結論として、レジオネラ抗体保有は病院環境中での職業暴露と関連すると考えられるが、疾患との関連を示すエビデンスは得られなかった。
