Clinical characteristics of nosocomial norovirus outbreaks in Hong Kong

ノロウイルスのアウトブレイクは世界各国で毎年発生しており、発生頻度はここ数年で増加している。2006年5月から7月に香港で広範なアウトブレイクが発生した。著者らの目的は、2006年5月1日から2006年7月31日の院内アウトブレイクの後向き観察研究を報告することである。患者218例、合計38件のノロウイルスアウトブレイク確定事例を特定した。患者の多くは高齢者で、平均年齢74.5歳（範囲3カ月～97歳）であった。そのうち62％は日常生活動作に全面介助もしくは部分介助が必要であり、83.9％は慢性基礎疾患を有し、56％に自力移動に制限があった。全体で97.2％の患者が下痢を呈したが、嘔吐があったのはそのうち46.3％のみであった。下痢の持続期間中央値は3日で、最長は24日であった。嘔吐の持続期間中央値は1日で、最長は15日であった。全患者の3分の1に発熱が認められた。RT-PCR法では患者の72.6％がノロウイルス陽性であった。結論として、ノロウイルスによる院内感染は自力移動に制限のある虚弱な高齢患者での発生が多く、これらの患者では症状の持続期間が長くなる可能性がある。
