著者らの血液部門で、大規模な病院建設作業と同時期に発生した侵襲性アスペルギルス症院内感染6例のアウトブレイク制御の成功を報告する。修正した感染制御策は、当部門の移設、建設現場での不浸透性バリアの設置、フェイスマスクの装着、および高リスク患者18例に対するボリコナゾール予防投与であり、これらの患者に侵襲性アスペルギルス症のブレイクスルー感染を発症した者はなかった。侵襲性アスペルギルス症院内感染アウトブレイクの予防には、臨床医、病院管理部門、技術部門、および建設部門による多角的な先行アプローチが不可欠である。

監訳者コメント：
易感染性宿主における防護環境の必要性は、造血幹細胞移植患者については明記されているが、それ以外の病態に関する知見は不足している。本事例はボリコナゾール予防投与まで行っており、かなりアグレッシブであるが、一時期の病棟閉鎖や転院などもファシリティが一定の基準を満たしていなければとるべき手段かもしれない。
